The Southern Grafton County data book is intended to be a resource for local organizations and community leaders in their efforts to monitor and improve family well-being and to encourage organizations and local leaders to develop policies, services, activities, outreach, and connections that will strengthen health promotion and risk prevention for children, youth and families.

The data book assembles a variety of information on the status of children, youth and families in the twenty-seven towns that comprise three core health regions:
Bridges to Prevention: The Community Prevention Coalition region of the NH Department of Health and Human Services' Strategic Prevention Framework serves 25 of the 27 communities in the Southern Grafton County area, focusing on strategic planning and evidence-based strategy capacity to address underage drinking and binge drinking among 12 to 34 year olds.
Southern Grafton County Health Consortium: This network of social service organizations and other providers covers 18 of the 27 communities in the region and engages in collaboration and information dissemination for a broad range of risk prevention and health promotion including early childhood and parent supports, mental health services, juvenile justice alternatives, and violence and substance abuse prevention.

- Voices Against Violence
- Whole Village Family Resource Center and Child Care Center
